If you are a patient of Trinity Surgery who is currently receiving a medical certificate from the GP and you require an extension on this, then you can use the online form below to request this.

Please note the following:

• Requests can take up to two working days to be actioned.
• Please only use this service if your medical certificate finishes today or before todays date as GP's are unable to post date these notes. If your request is received before your current note expires then you will receive a message asking you to request the note when it is due.
• Any medical certificates will be sent securely via text message to your mobile. If you require a paper printed copy instead, please state this on the online form at the time of your request.

This service is not to be used by patients who require their first medical certificate from the GP, this request will need to be made via telephone to the surgery once you have completed your 7 days self-certification.
